1)  Obtain the appropriate forms. One source of forms is the Superior Court Family Law Forms.

2)  When ready to file, bring the completed forms to room 131A at 110 W. Congress. Fill out the domestic cover sheet, which will be provided at our counter, and pay the filing fee which can be found online on this website under Filing Fees. If minor children are involved in this case, an additional parent education fee, which can be found online on this website under Filing Fees, will be required at this time.

3)  Submit the original Petition for Dissolution, Preliminary Injunction and Affidavit Regarding Minor Children for filing. Also, submit a copy of the affidavit with the preceding.

4)  Any copies of documents being filed, and/or originals and copies of documents to be filed later can be stamped with a case number and the name of the assigned judge upon request at this time.
